1) Please complete, sign the application form and refer to the application checklist for required documents stated in
the account application form.
2) Please complete and sign the following,
FINTECH BANK LTD. customer account agreement
Hamilton Reserve Bank customer account agreement
Personal Data Protection Act 2010 (PDPA) form
FATCA CRS Controlling Person Self-Certification form
Form W 9 if you are a U.S. Person or Legal Entity
Form W 8BEN if you are a non U.S. Natural Person
3) Email the account application form and required
supporting documents to FINTECH BANK
for assessment to email@example.com
4) Upon completion of assessment, the approved client is required to create bank account username and password online.
Please have these items ready before you begin:
- Your passport.
- A computer or mobile phone with a camera to take a selfie.
- A scanned image or PDF of Proof of address such as a utility/phone bill, credit card statement, rental agreement, etc.
The bank will email the bank account and wire transfer details.
The client is required to transfer a one time fee payment of USD3,000 to FINTECH BANK via SWIFT wire transfer.
