1) Please complete, sign the application form and refer to the application checklist for required documents stated in the account application form.
2) Please complete and sign the following,
FINTECH BANK LTD. customer account agreement
Hamilton Reserve Bank customer account agreement
Personal Data Protection Act 2010 (PDPA) form
FATCA/CRS Entity Self Certification form
Form W 9 if you are a U.S. Person or Legal Entity
Form W 8BEN if you are a non U.S. Natural Person
3) Email the account application form and required supporting documents to FINTECH BANK for assessment to firstname.lastname@example.org
Upon the bank’s assessment and approval of the business bank account application,
- the client will be notified via email to set up a username, password, and wire transfer instructions;
- one-time fee payment of USD3,000 via SWIFT wire transfer is required; and
- all original copies of the forms and documents are to be couriered to the bank’s office in Labuan, Malaysia within 4 weeks from the notification of the approval.
